I accept Auto/PIP. I will submit the claims directly to the insurance company to bill for services rendered.
Before your first visit, you will need :
- the date of injury
- a prescription from your primary care physician with his provider number (LNI or NPI).
- your claim number
- your claim manager name with phone number
- claims mailing address
- attorney information, if one has been retained
PIP
The insurance for the at-fault driver in your accident will be responsible for your medical bills. You will actually open a claim with your own auto insurance company, through your Personal Injury Protection (PIP) policy. The at-fault driver will then repay your auto insurance for PIP costs. You pay nothing to any of your medical providers when you use PIP coverage, and opening a PIP claim will not raise your rates.
